DAI Voluntary Self Identification Form - Completion of this form is voluntary and the data collected will be used solely for research and statistical purposes. The information provided will not affect opportunity for employment or the terms or conditions of employment if hired.

Contractor Employee Biodata Sheet (EBD) - This form is required from every employee or consultant who will be working on a U.S. Agency for International Development (USAID)-funded project. The proposed employee or consultant may refer to the following instructions and use the sample form as a guide.

Personal Data Form - The Personal Data Form is requested from consultants as part of an invitation to register in our Recruitment Management System (RMS). The information provided by the consultant is entered into a permanent profile and is easily circulated to all staff within DAI.
